Skip Clara, post-call feedback and clearer transfers

- Post-call feedback: rate how a call went the moment it ends, so we can keep tuning how Clara sounds and when she steps in.
- Always ask for the caller’s name: on urgent transfers Clara now confirms who is calling before she puts them through to you.
Improved
- Excluded Contacts is now Skip Clara: same feature, a name that says what it actually does.
- Live transfers sound clearer: you get a connection tone when a transfer goes through, plus a heads-up when the line quality drops.
- Transfer history is easier to read: unanswered attempts collapse into a single “No answer” line instead of one row per attempt.
